Buffy learns of Spike's life after his Shanshu & death

Does this sound familiar to anyone?

I think this is post-NFA. Spike shanshued, but he chose not to go to Buffy. Instead, he meets someone, marries, and has children.  After 10 years or so, he becomes ill, maybe cancer, and dies.

The story starts with Buffy who has just learned about Spike's short time as a human. She visits his wife and looks at pictures of Spike with his family.

I appreciate any help with this!

It's called His Final Death but I don't know the author unfortunately. I'll try to remember.
