Winchester/slayer children raise Buffy from the dead

I'm looking for a Supernatural/Buffy crossover series. My memory of the details are a bit foggy. Sam and Dean hook up with, I think, Buffy and Faith. They're trying to get out of a hospital (I think Buffy just had a baby?) which is infested with demons. Before they can get to the car, Buffy is stabbed and killed. Dean, Sam, Faith, and Willow(?) stay close as a family. Their children grow up together and have a lot of slayer/witch/hunter mystical power in them. Maybe ten or so years after Buffy's death the kids sneak out of the house to a nearby park and bring Buffy back.

*Whoops! Found it. It's been a bad morning and I got impatient in my search. As usual I find a story right after posting a search. The series is called To Live is to Die by Faith Winchester at Twisting the Hellmouth.
